8D Report

Full name: Eight Disciplines Problem Solving
An 8D report documents a structured, team-based problem-solving methodology used to identify, correct and prevent recurring or complex quality problems through eight defined steps, or disciplines. Developed by Ford Motor Company in the 1980s and first described in a 1987 Ford manual, the 8D process combines root cause analysis tools such as the 5 Whys, fishbone diagrams and Pareto analysis with structured containment, corrective action and prevention steps.

What is 8D Report?

The 8D methodology guides a cross-functional team through eight defined disciplines to solve a complex or recurring problem, from initial team formation through final closure and recognition. It is especially associated with the automotive industry, tracing back to Ford's development of the approach in response to costly, recurring vehicle quality issues in the 1980s.

  2. D2: Problem description. Clearly define the problem in specific, measurable terms.
  3. D3: Interim containment. Implement temporary actions to protect the customer while a permanent fix is developed.
  4. D4: Root cause analysis. Identify and verify the root cause of both occurrence and escape.
  5. D5: Corrective action selection. Choose and verify permanent corrective actions addressing the root cause.
  6. D6: Implementation. Implement and validate the corrective actions.
  7. D7: Prevention. Modify systems, practices or procedures to prevent recurrence on similar products or processes.
  8. D8: Closure. Recognize the team's efforts and formally close the report.
